Incident Reporting

Through this website any person involved directly or indirectly, at any level in the Design Organization of Daedalean AG, or as a third party, in the operation, installation, maintenance, manipulation, documentation or derivatives, should report any possible safety occurrence or information security incident to Daedalean AG.

This communication portal is accessible to anyone and the report will arrive to Daedalean’s SM (Safety Manager) and CISO (Chief Information Security Officer).

The information communicated will be treated as confidential, but a method of contact would be needed in case further information is required. 

After the submission of the report, the SM and/or CISO will assess the report and trigger the appropriate actions, provide feedback and apply the corrective measures to solve the problem.

The focus of this reporting mechanism is:

  • the commitment to aircraft safety (airworthiness), ensuring the continued safe operation of all Daedalean-equipped aircraft and,
  • the commitment to safeguard the confidentiality, integrity and availability of the physical and digital information from threats and unauthorized access within the organization.

Just Culture Declaration

The reporter does not experience any prejudice based on the information they have reported to Daedalean AG, in line with the reporting requirements. This principle applies, as far as no willful misconduct or serious lack of professional duty of care is evident.

Please provide the product or part affected by the event. This will include things (but not limited to) aircraft, maintenance (equipment or procedures), operations, personal, installations, facilities, documentation, etc. 
Provide a detailed description of the issue, including fault codes and context, and attach all relevant supporting materials with your submission. For concessions, also include the affected part number, quantity, and time of manufacture or maintenance.

Note: Only the fields marked with an asterisk (*) are required. Confidentiality and anonymity is guaranteed. In case the reporter needs to provide more data or is interested about the progress of her/his query, a means of contact (email or phone) will be needed. This means of contact is helpful for potential, further clarifications. Regardless, confidentiality will be ensured as only the the SM or CISO will receive the report and they are committed and trained to treat this information with strict confidentiality.
